The Annual Physical: What It Should Include and What It Usually Misses

The typical annual physical takes about 15 minutes once you are actually in the room with the provider. Vitals get checked, a stethoscope passes over your chest and back, you might get asked a few questions about how you have been feeling, and labs are ordered using whatever your insurance covers as standard. For most people in their 30s and 40s, that lab order includes a basic metabolic panel, a CBC, a TSH, and a lipid panel. By default, that becomes the version of preventive care you receive year after year, and most people assume it covers what needs to be covered. It usually does not.

The annual physical is one of the most underutilized tools in preventive medicine, not because the concept is flawed but because the standard version is often too abbreviated to catch the conditions it should detect before symptoms develop. A brief exam, a basic metabolic panel, and a referral for a mammogram or colonoscopy leaves several significant diagnostic gaps that a more complete evaluation would close. This post covers what a genuinely useful annual visit should include, why those additions matter, and how to ask for them at your next appointment.

What a Standard Annual Physical Actually Includes

The standard annual physical typically includes a blood pressure check, a basic or comprehensive metabolic panel, a CBC, TSH (thyroid-stimulating hormone) alone, and a lipid panel. For women over 40, a mammogram referral is standard. For appropriate-age patients, a colonoscopy recommendation. This is a reasonable minimum for catching obvious problems, but it misses several high-yield tests that change clinical decisions when added.

The reason these limited panels are standard has more to do with insurance coverage and time pressure than with what would be ideal preventively. A primary care visit allotted 15 to 20 minutes cannot review nuanced lab results, take a thorough hormone history, and discuss preventive strategy in meaningful depth. The labs ordered tend to be the labs that fit comfortably inside that constraint. To get the more complete picture, you usually have to ask for it specifically, or see a provider whose practice is built around offering that more complete panel as the default.

The Tests Most Annual Physicals Miss

A more complete preventive evaluation should add five tests that meaningfully change what a clinician actually knows about your health. None of them is exotic. All of them are widely available. Most insurance plans cover them when ordered with appropriate clinical justification. The reason they do not show up on standard panels has more to do with what gets requested than what is technically available.

  • Fasting insulin alongside fasting glucose. The combination allows calculation of HOMA-IR, which detects insulin resistance significantly earlier than fasting glucose alone. Insulin resistance precedes type 2 diabetes by 10 to 15 years, and the intervention window during early resistance is far more effective than after diabetes develops. A National Institute of Diabetes and Digestive and Kidney Diseases overview notes that insulin resistance is reversible with lifestyle intervention at early stages.
  • HbA1c in addition to fasting glucose. HbA1c reflects average blood glucose over the previous three months, providing a longitudinal picture rather than a single morning snapshot, and detects glucose dysregulation that fasting values alone may miss.
  • Full thyroid panel including free T3, free T4, and thyroid peroxidase antibodies (TPO-Ab), not just TSH. TSH alone misses early autoimmune thyroiditis (Hashimoto’s), which can produce fatigue, weight changes, and cognitive symptoms with a normal TSH for years before TSH becomes abnormal.
  • Vitamin D (25-OH vitamin D). Deficiency affects an estimated 40 percent of adults in the United States and contributes to immune dysfunction, fatigue, bone density loss, and mood disorders. A meta-analysis in the BMJ found that vitamin D supplementation reduced respiratory infection risk in deficient individuals.
  • High-sensitivity CRP as a cardiovascular and systemic inflammation marker independent of lipids. Elevated hsCRP predicts cardiovascular events at levels where standard cholesterol panels look unremarkable, which is why it adds information rather than duplicating what the lipid panel already shows.

The Hormone Evaluation Gap After Age 40

Standard annual physicals do not routinely include hormone evaluation. For patients over 40, this is a significant gap. Testosterone, estradiol, progesterone, DHEA-S, AM cortisol, and prolactin all provide clinical information relevant to fatigue, mood, body composition, cognitive function, libido, and long-term bone and cardiovascular health. Many of the symptoms patients attribute to stress, aging, or depression are partly or fully hormone-driven and respond meaningfully to appropriate treatment when the underlying picture is identified.

The argument against universal hormone screening is that treatment decisions require clinical context, not just a number, and that argument has merit. A testosterone level near the lower end of the reference range may be adequate for one man and clinically deficient for another, depending on symptoms, trajectory, and individual baseline. But a screening panel reviewed in clinical context is exactly what a thoughtful provider should be doing, and that requires having the panel in the first place. Without it, the conversation cannot happen, and the patient never knows whether hormones are part of what they have been feeling.

Patients in their 40s and 50s who quietly accept that they “just do not feel like themselves anymore” are often dealing with hormone changes that would be addressable if anyone thought to check. The cost of including a hormone panel at the annual visit is small. The cost of missing it for years is the cumulative experience of feeling worse than you should, attributed to aging, and never investigated.

Why Lab Reference Ranges Are Not Health Targets

Lab reference ranges are statistically derived. They describe where roughly 95 percent of values from a presumed-healthy population fall. They are not derived from studies of optimal health. A value at the low end of a reference range is statistically “normal” without meaning much about whether it is optimal for the person being tested.

The clearest example is testosterone. The reference range for adult men spans roughly 300 to 1,000 ng/dL in many labs. A 45-year-old man with a level of 320 ng/dL is technically “normal” by the reference range, even if he has fatigue, low libido, declining muscle mass, and a level that was 750 ng/dL when last measured in his 20s. Reading the result as “normal” and stopping the conversation there misses the clinical reality of the trajectory.

The same pattern applies to vitamin D, ferritin, free T3, and several other markers. Reference ranges are useful for excluding gross abnormality. They are insufficient for identifying the slow declines that drive most chronic disease and most everyday symptoms. Clinical interpretation, with attention to where you are within the range and how that compares to where you have been, is what makes lab work genuinely informative rather than just reassuring.

What to Ask For at Your Next Annual Physical

If switching providers is not practical, you can still ask your current primary care provider to add specific tests to your annual labs. Most reasonable clinicians will order them when patients ask with specific clinical context. The framing that tends to work best is something like this: “Given my age, family history, weight goals, and the fatigue I have been dealing with, could we add fasting insulin, a full thyroid panel including free T3 and TPO antibodies, vitamin D, and a hormone panel to this year’s labs?”

For the hormone panel specifically, men should ask for total testosterone, free testosterone, and SHBG at minimum. Women in their 40s and 50s should ask for estradiol, progesterone, FSH, and total testosterone. DHEA-S and AM cortisol round out the picture for both. None of these tests is unusual or difficult to order.

Insurance coverage varies by plan and by clinical justification documented at the visit. Some plans cover these tests routinely; others require them to be self-pay. A self-pay full panel typically runs in the low hundreds of dollars depending on local pricing, which is not trivial but is meaningful information that informs the next decade of preventive decisions.

What a More Useful Preventive Visit Looks Like

That framing changes what a preventive visit looks like in practice, and it is what many patients say they wished their annual physical had been all along.

On insurance coverage and self-ordering: Most of the tests in this post are covered by insurance when ordered with appropriate clinical justification at a routine visit. For patients whose insurance does not cover them, direct-to-consumer testing through major labs is also an option, typically in the low hundreds of dollars for a comprehensive panel including hormones. The cost is meaningful but the information shapes preventive decisions for years.

Sex hormone binding globulin. A protein that binds testosterone and estrogen in the bloodstream. SHBG levels affect how much testosterone is biologically available, which is why total and free testosterone can tell different stories.
