What Is Semaglutide, and How Does It Work?

Semaglutide is a GLP-1 receptor agonist, a synthetic version of a naturally occurring hormone (glucagon-like peptide-1) that regulates appetite, blood sugar, and gastric emptying. By binding to GLP-1 receptors in the brain, it reduces hunger signals, increases the feeling of fullness after eating, and slows the rate at which food leaves the stomach.

What is the difference between Ozempic, Wegovy, and compounded semaglutide?

Ozempic and Wegovy contain the same active ingredient (semaglutide) but are FDA-approved for different indications. Ozempic is approved for type 2 diabetes, and Wegovy is approved for chronic weight management at the higher 2.4mg dose. Compounded semaglutide contains the same molecule, prepared by licensed 503A/503B compounding pharmacies, and is prescribed when brand-name products are unavailable or inaccessible. Compounded medications are not FDA-approved products. Our clinical team will recommend the right option based on your clinical situation and access.

How does the semaglutide titration schedule work at Tactus Health?

We start at 0.25 mg weekly for four weeks to minimize gastrointestinal side effects during the titration phase. Your providers then increase your dose at monthly check-ins based on your tolerance and response, not a fixed schedule. The goal is to reach the most effective dose for you without the nausea and discomfort that cause most patients to abandon GLP-1 therapy. Patients who titrate slowly consistently report better experiences and better long-term results.

Do I need labs before starting semaglutide?

Labs are recommended but not always required. Baseline bloodwork helps our clinical team personalize your protocol and identify anything that would affect your treatment. If you have labs completed within the past 90 days, those are typically sufficient. Our providers will advise during your consultation on whether new labs are needed in your specific case. For telehealth patients, Quest Diagnostics and Labcorp have thousands of convenient locations nationwide.

What are the side effects of semaglutide?

The most common side effects are gastrointestinal: nausea, constipation, and occasionally vomiting or diarrhea. These are most common during dose escalation and typically improve as your body adjusts. The conservative GLP-1 titration schedule at Tactus Health dramatically reduces the severity of these effects compared to aggressive dose increases. More serious but rare risks include pancreatitis. An absolute contraindication exists for those with a personal or family history of medullary thyroid cancer.

Will I regain weight when I stop semaglutide?

Clinical data from the STEP 4 trial shows that patients who discontinued semaglutide after significant weight loss regained approximately two-thirds of their lost weight within a year. Obesity is a chronic condition. Semaglutide manages it rather than curing it, similar to how blood pressure medication manages hypertension. Long-term use planning, and in some cases transitioning to a lower maintenance dose, are part of how your care team approaches this conversation.

Can I take semaglutide if I have type 2 diabetes?

Yes. Semaglutide was originally developed for type 2 diabetes management. If you have T2D, semaglutide may be especially beneficial as it addresses both blood sugar control and weight.
