Contrave® combines naltrexone (opioid antagonist) and bupropion (NDRI) in a single extended-release tablet, FDA-approved for chronic weight management since 2014. It targets both the reward and satiety pathways simultaneously, making it most effective for patients whose eating is driven by emotional cues, boredom, or habit.
Why the Combination Works Better Than Either Drug Alone
Contrave® (naltrexone 8mg / bupropion 90mg extended-release) is FDA-approved for chronic weight management in adults with a BMI of 30 or higher, or 27 or higher with at least one weight-related comorbidity. It works through two distinct and complementary neurological pathways: bupropion activates POMC neurons in the hypothalamus, stimulating the hypothalamic appetite centers to reduce hunger and increase energy expenditure. Naltrexone blocks the auto-inhibitory opioid feedback that would otherwise limit POMC neuron activity, allowing bupropion’s effect to be sustained rather than diminishing over time. By simultaneously stimulating the hypothalamic appetite centers and silencing the dopaminergic reward system, Contrave® treats both the physical and emotional triggers of weight gain in a single daily tablet.
In the COR clinical trial program, Contrave® produced average weight loss of 6.1–9.3% of body weight at 56 weeks versus 1.3% with placebo. This is clinically meaningful, though significantly less than the 15–20% achievable with GLP-1 medications. Contrave® is most appropriate for patients who cannot tolerate GLP-1 GI side effects, who prefer an oral-only protocol without injections, or who have specific emotional or reward-driven eating patterns that its dual mechanism targets well.
For patients where cost is a factor, the individual generic components (naltrexone + bupropion) can be prescribed separately at significantly lower cost than brand-name Contrave®, producing the same combination at a fraction of the price. The titration schedule and monitoring protocol remain the same.
For patients managing demanding schedules, Contrave® is a discreet, travel-friendly oral tablet that requires no refrigeration and no logistical management of weekly injections.
While the Wegovy® pill is the newest oral option, Contrave® remains the most established and widely used FDA-approved oral weight loss medication for our telehealth patients, with over a decade of real-world safety data.

Generic naltrexone and generic bupropion can alternatively be prescribed separately, which allows for more flexible dosing and typically costs significantly less. Contrave® FAQ
GLP-1 medications (semaglutide, tirzepatide) produce significantly greater average weight loss, 15–20% versus 6–9% for Contrave®. Contrave® is preferred for patients who cannot tolerate GLP-1 GI side effects, who prefer an oral-only protocol without injections, or who have emotional or reward-driven eating patterns that GLP-1 alone does not fully address. Contrave® is titrated over 4 weeks to reduce GI and neurological side effects: week 1, one tablet in the morning; week 2, one tablet twice daily; week 3, two tablets in the morning plus one in the evening; week 4 onward, two tablets twice daily as the maintenance dose. The gradual titration is the main reason patients stay on Contrave® long enough to see results. Patients who skip titration experience significantly more nausea and headache.
Coverage varies by plan. As a direct-pay practice, Tactus Health does not bill insurance. However, generic naltrexone and generic bupropion can be prescribed separately, achieving the same combination at a fraction of brand-name Contrave® cost. Both generics are widely available at retail pharmacies. The dosing protocol and monitoring remain the same regardless of which version is prescribed.
Warning: Contrave® contains naltrexone and will cause sudden, severe opioid withdrawal if taken with opioid pain medications. Taking it while on opioid medications is an absolute contraindication. You must be fully opioid-free (typically 7–10 days minimum) before starting Contrave®. A full medication reconciliation is required before prescribing to confirm this and all other contraindications are addressed.
Contrave® carries a boxed warning for suicidal thoughts and behaviors in patients under 25. Contraindicated in patients with uncontrolled hypertension, seizure disorders, eating disorders (anorexia or bulimia), current opioid use (precipitates severe withdrawal), abrupt alcohol or sedative withdrawal, and concurrent MAOI use. Common side effects include nausea, constipation, headache, vomiting, insomnia, and dry mouth. Risk of seizure increases with dose. Discontinue gradually under medical supervision.
