TRT and Blood Pressure: What to Watch

Blood pressure is one of the specific cardiovascular parameters that can shift on TRT. Most men see modest changes that do not require intervention. A minority see meaningful increases that warrant attention. And men on oral testosterone undecanoate (Jatenzo) have a specific blood pressure warning in the FDA label because of clinical trial findings. This post covers how TRT typically affects blood pressure, why oral TRT is different, and what to do if your numbers start drifting up.

TRT and Blood Pressure: How TRT Can Affect You

Blood pressure on TRT is not governed by one single mechanism. Several factors contribute, and they operate in different directions.

Potential mechanisms that can raise BP:

  • Water and sodium retention. Testosterone and its estradiol metabolite can cause modest fluid retention, particularly early in treatment
  • Increased red blood cell mass. Higher hematocrit increases blood volume and viscosity
  • Sympathetic nervous system activation. Higher testosterone may increase sympathetic tone in some men
  • Sleep disruption. If TRT affects sleep quality, BP can rise secondarily

Potential mechanisms that can lower BP:

Several mechanisms work in favor of cardiovascular health on TRT: Improved endothelial function. Testosterone supports nitric oxide signaling and vascular relaxation. Reduced visceral fat. Over months to years, TRT tends to reduce metabolically active fat that contributes to hypertension. Better metabolic health. Improved insulin sensitivity is cardiovascularly protective.

The net effect depends on which mechanisms dominate in a particular man. On average, most men see small BP changes that do not require specific intervention.

What the Evidence Shows on TRT Blood Pressure Effects

Data on BP effects comes from several sources.

TRAVERSE trial. The large 2023 cardiovascular safety trial tracked blood pressure as a secondary outcome. No meaningful differences in sustained blood pressure between the testosterone gel and placebo arms. This applies to transdermal and implicitly to injectable TRT.

Injectable TRT studies. Most studies show modest, variable effects. Average systolic BP changes of 0 to 3 mmHg. Some men see no change; a minority see larger increases.

Oral testosterone trials. The Phase 3 trials of Jatenzo specifically measured ambulatory blood pressure and documented an average systolic increase of 3 to 5 mmHg. This signal was strong enough that the FDA added a specific BP warning to the oral testosterone undecanoate label.

The difference between oral and injectable/transdermal is likely related to how oral testosterone is absorbed and how the body processes its metabolites. Whatever the mechanism, the clinical implication is that oral TRT warrants closer BP monitoring than the other delivery methods.

Why Oral TRT Is Different

Oral testosterone undecanoate is absorbed via the lymphatic system, bypassing first-pass liver metabolism. This was the pharmacologic achievement that made modern oral TRT possible. However, the delivery produces different tissue exposure patterns than injections or gels, including effects on blood pressure regulation that have not been fully characterized mechanistically.

What this means for patients:

  • Men starting Jatenzo or Kyzatrex need BP checked more often than men on injectables
  • Men with pre-existing hypertension should have BP well-controlled before starting oral TRT

The FDA label requires BP monitoring at baseline, 3 weeks, 6 weeks, 3 months, and every 6 to 12 months thereafter for men on oral testosterone. Men on injectables do not have this specific label requirement though clinical practice often follows a similar schedule.

Baseline BP Evaluation Before Starting TRT

Dr. Ashar’s approach to BP evaluation at TRT initiation:

  1. Office BP on day of consultation. Two readings 5 minutes apart.
  2. Home BP log. Ask patients to check BP at home morning and evening for 7 to 14 days before starting TRT. This captures the real trend and removes white coat effect.
  3. Review medications. Some medications (NSAIDs, decongestants, some antidepressants) raise BP. Optimize non-TRT contributors first.
  4. Lifestyle review. Sodium intake, alcohol, exercise, sleep. Address modifiable factors.

If baseline BP is consistently in stage 2 hypertension range (140/90 or higher) despite medications, TRT is generally deferred until BP is better controlled. Starting TRT on top of uncontrolled BP makes management harder and complicates the ongoing picture.

Men with well-controlled hypertension on medications can start TRT with closer BP monitoring as part of the follow-up schedule.

Monitoring BP on TRT

Standard BP monitoring on TRT:

  • Baseline: Office readings plus home log as described above
  • 6 weeks on TRT: Office BP at follow-up visit, plus recent home log if available
  • 3 months: Office BP plus home log review
  • Every 6 months thereafter: Office BP at minimum, home log for men with prior BP issues or on oral TRT
  • Any symptoms suggesting BP issue: Immediate recheck (headaches, visual changes, dizziness)

Home BP monitoring has become significantly more accurate and affordable. A validated upper-arm cuff costs 30 to 100 dollars and provides far more useful data than periodic office readings alone. The American Urological Association testosterone deficiency guideline specifically recommends routine blood pressure evaluation as part of TRT follow-up, with tighter monitoring in men on oral testosterone or with pre-existing hypertension.

When BP Rises on TRT

If your BP climbs on TRT, the action sequence:

Confirm the trend. One high reading is not a trend. Document multiple readings over a week to verify.

Review contributors. Sodium intake, alcohol, stress, sleep, medications. A recent change in any of these may explain the shift.

Check hematocrit. Elevated hematocrit increases blood viscosity and BP. If hematocrit is above 52 percent, reducing it (via dose reduction, formulation change, or phlebotomy) often resolves BP.

Check estradiol. High E2 contributes to fluid retention which affects BP. Managing estradiol can help.

Consider TRT dose reduction. If labs and symptoms suggest dose is on the high end for your physiology, reducing often normalizes BP.

Consider formulation switch. Men on oral TRT with BP issues can consider switching to injectable or transdermal. BP response often improves.

Add or adjust antihypertensive therapy. Some men simply need a BP medication (or an adjustment to existing therapy) alongside TRT. This is not a failure; it is a combination approach to two related issues.

The Endocrine Society Clinical Practice Guideline on Testosterone Therapy specifically addresses blood pressure monitoring during TRT and recommends evaluation of contributing factors before attributing BP changes solely to the testosterone medication.

TRT in Men With Pre-Existing Hypertension

Men with controlled hypertension can generally take TRT safely. The common combinations:

Controlled on single medication: Usually fine. Continue medication, monitor BP per schedule, adjust if needed.

Controlled on multiple medications: Fine with closer monitoring. Coordination with primary care or cardiology for BP management continues alongside TRT.

Resistant hypertension (3+ medications): Evaluate secondary causes (sleep apnea, aldosterone excess, renal artery stenosis) before adding TRT. Do not simply add TRT on top of difficult-to-control BP.

Hypertensive urgency/emergency: Must be resolved and stable before TRT consideration.

The White Coat Effect and BP Measurement

Office BP readings are often higher than home readings due to the white coat effect (anxiety from medical settings). This matters on TRT because office readings can give a falsely alarming picture.

Proper BP measurement technique:

  • Seated quietly for 5 minutes before measurement
  • Feet flat on floor, back supported
  • Arm at heart level, cuff over bare skin
  • No caffeine, alcohol, or tobacco 30 minutes prior
  • Two readings 2 minutes apart; use the second
  • Same arm every time

Home BP logs taken properly are usually more reliable than office readings for trending. Bring the log to follow-up visits.

Medical note: Severe sudden BP elevation (greater than 180/120) with or without symptoms warrants immediate medical attention, regardless of TRT status. Hypertensive urgency or emergency is a medical emergency requiring prompt evaluation.

The Blood Pressure Summary

TRT usually does not dramatically affect blood pressure, but it can. Oral TRT has a specific BP warning based on trial data. Men with well-controlled hypertension can usually take TRT with closer monitoring. Men with uncontrolled hypertension should optimize BP before starting TRT. Home BP monitoring provides the most useful data for tracking. When BP does rise on TRT, systematic evaluation of contributing factors usually identifies what to adjust, whether that is TRT dose, formulation, or non-TRT factors like sleep and sodium.
