Since then, Kyzatrex and Tlando have joined the market, and oral TRT is a legitimate option for men who want to avoid injections.

Oral Testosterone vs Injection: How Each Delivery Method Works
The two delivery routes differ in where the testosterone enters your bloodstream and how quickly it clears, which shapes everything else about how they perform clinically.
Injectable Testosterone
Injectable testosterone (usually cypionate or enanthate) is dissolved in an oil carrier and injected into muscle or subcutaneous fat. The ester bond slows absorption, releasing testosterone gradually over days. Blood levels peak 24 to 48 hours after injection and trough 5 to 8 days later. Weekly or twice-weekly dosing maintains therapeutic levels across time.
The testosterone goes directly from the injection site into the systemic circulation. The liver eventually metabolizes it, as it does any circulating hormone, but it does not receive a concentrated first-pass dose that would cause damage.
Oral Testosterone (Modern)
The problem with traditional oral testosterone was that when testosterone is absorbed through the stomach and small intestine, it goes through the portal circulation to the liver first (the “first-pass effect”). The liver metabolizes most of it before it reaches systemic circulation. To get a therapeutic dose systemically, the old protocol required enough oral testosterone that the liver took significant damage over time.
Modern oral testosterone solves this with lymphatic absorption. Testosterone undecanoate (the active ingredient in Jatenzo, Kyzatrex, and Tlando) is highly lipid-soluble and is taken up by the lymphatic system when consumed with fat. The lymph system bypasses the liver and delivers testosterone directly to systemic circulation. This is a significant pharmacological achievement. Cost. A vial of testosterone cypionate costs 30 to 80 dollars for a 10 ml supply that lasts most patients 10 to 16 weeks. Jatenzo and Kyzatrex typically cost 300 to 500 dollars per month at retail, though insurance coverage varies. For cash-pay TRT patients, injections are dramatically cheaper.
Dosing flexibility. Injection dose can be adjusted in 10 mg increments simply by drawing less or more into the syringe. Oral testosterone doses are fixed by the capsule strength.

The Case for Oral Testosterone
Oral testosterone genuinely solves problems for certain patients, and it deserves fair consideration where it fits.
No needles. For patients with severe needle phobia, certain autoimmune conditions, or professions that make self-injection impractical, oral TRT removes a real barrier. This benefit alone makes it the right choice for a subset of patients.
Smoother blood levels within the day. Because of twice-daily dosing, oral TRT produces less day-to-day variation than weekly injections. The trade-off is more day-to-day management (remembering doses with meals).
No injection site issues. Some patients develop persistent injection site irritation, lumps, or skin changes. Oral TRT sidesteps this entirely.
Privacy. Some patients prefer a pill bottle in the medicine cabinet over testosterone vials and needles. This is not a medical reason, but it is a real-world consideration.
Travel convenience. A pill bottle goes through security easily. Carrying vials, syringes, and needles across state lines or internationally is more complicated, particularly given that testosterone is a controlled substance.
Safety Profile Comparison
The safety concerns of injectable and oral TRT are mostly the same because both deliver the same hormone. A few differences are worth noting.
Blood pressure with oral testosterone. Jatenzo’s FDA label specifically warns about blood pressure elevation. Clinical trials showed that oral testosterone undecanoate raised systolic blood pressure by approximately 5 mmHg on average. Patients with pre-existing hypertension may not be good candidates, or may need closer BP monitoring. Injectable testosterone has less consistent BP effects. Liver toxicity. Modern oral testosterone does not cause the hepatotoxicity that older oral testosterone did. Studies on Jatenzo, Kyzatrex, and Tlando have not shown liver enzyme elevations in long-term use. Older warnings about oral testosterone and liver damage apply to methyltestosterone and similar 17-alpha-alkylated androgens, not to modern testosterone undecanoate.
Erythrocytosis. Oral testosterone appears to cause less hematocrit elevation than injections. For patients who have struggled with high hematocrit on injections, switching to oral can sometimes resolve the issue.
Estradiol. Conversion to estradiol occurs with both delivery methods. Oral testosterone may produce different estradiol kinetics because of the altered absorption route. Monitoring is the same.
Testicular suppression and fertility. Both delivery methods suppress endogenous testosterone production equally. Neither preserves fertility on its own. Men on either TRT who want to maintain fertility should discuss HCG with their prescriber.
Transdermal Gels: A Third Option
No discussion of TRT delivery is complete without mentioning transdermal gels like AndroGel, Testim, and Fortesta. These deliver bioidentical testosterone through the skin in daily applications. They avoid needles and are more affordable than oral options, though still more expensive than injections.
The main concerns with gels:
- Transfer risk. The gel can transfer to partners and children through skin contact before it fully absorbs. Warning labels require 6+ hours between application and close contact with others.
- Absorption variability. Skin thickness, body composition, and application site affect absorption. Some patients absorb poorly and cannot reach therapeutic levels at maximum doses.
- Daily compliance. Missing a day meaningfully lowers blood levels.

Which Is Right for You?
The right TRT delivery depends on your specific situation. The decision framework:
Choose injections if: You are cost-conscious, want the most stable lab-monitored protocol, do not mind self-injection, or need dose flexibility beyond what fixed-dose products offer. This fits most patients.
Choose oral TRT if: You have a strong needle aversion that injections cannot overcome, cost is not a primary concern, and you can reliably take medication with fatty meals twice daily. Rule out pre-existing hypertension as a contraindication.
Choose transdermal gel if: You want to avoid needles, cannot manage oral TRT’s dosing-with-food requirement, and do not have young children or partners who would be at risk from skin transfer. Consider absorption adequacy with lab monitoring.
