Our Duty to Safeguard Your Protected Health Information

Tactus Health is required by law to maintain the privacy of your Protected Health Information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to notify you following a breach of your unsecured PHI. We are required to abide by the terms of this Notice as currently in effect.

How We May Use and Disclose Your PHI

We use and disclose your PHI for the following purposes without requiring your written authorization:

Treatment: Sharing your information with other healthcare providers (labs, pharmacies, specialists) involved in your care
Payment: Processing billing and payment for services rendered
Healthcare Operations: Quality assessment, training, compliance, and administrative functions
As Required by Law: Reporting to public health authorities, responding to legal process, or other legally mandated disclosures
Health Oversight: Government audits, investigations, and inspections
Emergency Situations: Disclosures necessary to prevent serious threat to health or safety

Uses Requiring Your Authorization

We will obtain your written authorization before using or disclosing your PHI for purposes other than those described above, including:

Most uses and disclosures of psychotherapy notes
Uses and disclosures for marketing purposes
Sale of your PHI
Other uses not permitted without authorization

Your Rights Regarding Your PHI

You have the following rights with respect to your PHI:

Right to Access: You may request a copy of your medical records. We will provide access within 30 days of your request.
Right to Amend: You may request correction of inaccurate or incomplete information in your record.
Right to an Accounting: You may request a list of certain disclosures we have made of your PHI.
Right to Restrict: You may request that we limit how we use or disclose your PHI, though we are not required to agree to all restrictions.
Right to Confidential Communications: You may request that we communicate with you in a particular way or at a particular location.
Right to a Paper Copy: You may request a paper copy of this Notice at any time.
Right to be Notified of Breach: You have the right to be notified if your unsecured PHI is breached.

Right to File a Complaint

If you believe your privacy rights have been violated, you may file a complaint with Tactus Health or with the U.S. Department of Health and Human Services Office for Civil Rights. Changes to This Notice
