Bioidentical vs. Synthetic: What Actually Changes
Not all hormone therapy is the same. The molecule matters, and here is why.
Bioidentical hormones share the exact same molecular structure as the hormones your body makes. Estradiol is estradiol. Progesterone is progesterone. When a bioidentical hormone binds to a receptor, it signals the same way your body’s own hormone would, because it is structurally identical to it. This is the clinical foundation of bioidentical hormone therapy at Tactus Health.
Synthetic hormones work differently. Medroxyprogesterone acetate (MPA), the synthetic progestin used in the original 2002 WHI study, binds to progesterone receptors but also activates glucocorticoid and androgen receptor pathways that natural progesterone does not. Conjugated equine estrogens, also used in the WHI, contain estrogens not naturally present in the human body. Same receptor targets, very different downstream effects, and a measurably different risk profile.
Our medical team uses bioidentical estradiol and micronized progesterone in both standardized (FDA-approved) and precision-compounded formulations. The choice is driven by your labs and your biology, not by a preset hierarchy.
We work exclusively with 503A and 503B FDA-registered compounding pharmacies that provide certificates of analysis for every formulation. We do not work with unregulated compounders or pellet mills that cannot document accuracy and sterility.

The WHI Study Did Not Study Bioidentical HRT.
The 2002 Women’s Health Initiative used conjugated equine estrogens and medroxyprogesterone acetate in women averaging 63 years of age, more than a decade past menopause. The findings were real. But they applied to that specific population using those specific synthetic hormones. Transdermal bioidentical estradiol and micronized progesterone were not part of the study. The decades of undertreated women that followed represent one of the most consequential failures of evidence translation in modern medicine.

Low-Dose Testosterone for Women: Why It Matters
Women produce testosterone in the ovaries and adrenal glands. Levels decline with age and drop further with surgical menopause. When testosterone falls, it shows up as fatigue, low libido, cognitive fog, and muscle loss, symptoms that estrogen alone often does not fix.
No FDA-approved testosterone product exists for women in the US. We use precision-dosed compounded creams to safely restore libido and cognitive drive without androgenic side effects. The dose is 10 to 20 times lower than male TRT, calibrated to physiologic female levels confirmed by lab work.

More precisely: specific bioidentical formulations have a more favorable risk profile than the specific synthetic formulations studied in the WHI. Transdermal estradiol carries no statistically significant increased VTE risk. Micronized progesterone carries a lower breast cancer signal than medroxyprogesterone acetate. The distinction between “bioidentical” and “conventional” matters, but the delivery route and progestogen choice matter equally. Both can be bioidentical. FDA-approved options are standardized, batch-tested, and widely available. Compounded formulations allow for precision dosing, combined hormones, and concentrations that standard products do not offer. FDA-approved bioidentical hormones are often covered by insurance plans with a valid diagnosis. Estradiol patches (Climara, Vivelle-Dot) and micronized progesterone (Prometrium) have generic versions covered by most major plans. Compounded BHRT is generally cash-pay because insurance does not cover custom compounded medications. HSA and FSA accounts are accepted for all hormone therapy services at Tactus Health.
