Androgenetic alopecia, the most common form of male pattern hair loss, is caused by dihydrotestosterone (DHT) progressively miniaturizing hair follicles in the scalp. Over time, affected follicles produce thinner, shorter hairs until they stop producing hair altogether. Shampoos, supplements, and topical treatments that do not address DHT do not stop this process. Finasteride is a 5-alpha reductase inhibitor that reduces DHT production at the source. Combined with minoxidil, which extends the follicle growth phase and increases scalp blood flow, it is the most evidence-backed medical treatment available.

Finasteride and Minoxidil: How They Work Together
Finasteride and minoxidil work through completely different mechanisms, which is why combining them is more effective than either alone. Finasteride inhibits the enzyme that converts testosterone to DHT in the scalp, reducing DHT levels and stopping the hormonal signal that causes follicle miniaturization. The effect is at the root cause of the hair loss process.
Minoxidil works at the follicle level by extending the anagen (growth) phase and promoting vasodilation to improve blood flow and nutrient delivery to the scalp. It does not address DHT, but it supports follicle activity and can accelerate regrowth alongside finasteride. Available in oral and topical formulations, with your provider advising on which form fits your situation.
Treatment requires consistency. Neither medication produces meaningful results without daily use over a minimum of 3 to 6 months. Starting earlier, before significant follicle miniaturization, produces better outcomes. Follicles that are fully miniaturized cannot be fully restored, so the goal is to stop progression and recover what is recoverable.

The key variables are starting before advanced miniaturization has occurred and using it consistently. It does not work if not taken daily.
The most discussed potential side effects are sexual in nature: decreased libido, erectile dysfunction, and decreased ejaculatory volume.
